Medical: General Knowledge about Dialysis


Dialysis is a treatment that does some of the things done by healthy kidneys. It is needed when your own kidneys can no longer take care of your body's needs.

When is dialysis needed?

You need dialysis when you develop end stage kidney failure --usually by the time you lose about 85 to 90 percent of your kidney function and have a GFR of <15. Click here to learn more about the stages of Chronic Kidney Disease and GFR.

What does dialysis do?

When your kidneys fail, dialysis keeps your body in balance by:

  • removing waste, salt and extra water to prevent them from building up in the body
  • keeping a safe level of certain chemicals in your blood, such as potassium, sodium and bicarbonate
  • helping to control blood pressure

Is kidney failure permanent?

Usually, but not always. Some kinds of acute kidney failure get better after treatment. In some cases of acute kidney failure, dialysis may only be needed for a short time until the kidneys get better.

In chronic or end stage kidney failure, your kidneys do not get better and you will need dialysis for the rest of your life. If your doctor says you are a candidate, you may choose to be placed on a waiting list for a new kidney.

Where is dialysis done?

Dialysis can be done in a hospital, in a dialysis unit that is not part of a hospital, or at home. You and your doctor will decide which place is best, based on your medical condition and your wishes.

Are there different types of dialysis?

Yes, there are two types of dialysis --hemodialysis and peritoneal dialysis.

What is hemodialysis?

In hemodialysis, an artificial kidney (hemodialyzer) is used to remove waste and extra chemicals and fluid from your blood. To get your blood into the artificial kidney, the doctor needs to make an access (entrance) into your blood vessels. This is done by minor surgery to your arm or leg.

Sometimes, an access is made by joining an artery to a vein under your skin to make a bigger blood vessel called a fistula.

However, if your blood vessels are not adequate for a fistula, the doctor may use a soft plastic tube to join an artery and a vein under your skin. This is called a graft.

Occasionally, an access is made by means of a narrow plastic tube, called a catheter, which is inserted into a large vein in your neck. This type of access may be temporary, but is sometimes used for long-term treatment.

How long do hemodialysis treatments last?

The time needed for your dialysis depends on:

  • how well your kidneys work
  • how much fluid weight you gain between treatments
  • how much waste you have in your body
  • how big you are
  • the type of artificial kidney used

Usually, each hemodialysis treatment lasts about four hours and is done three times per week.

A type of hemodialysis called high-flux dialysis may take less time. You can speak to your doctor to see if this is an appropriate treatment for you.

What is peritoneal dialysis and how does it work?

In this type of dialysis, your blood is cleaned inside your body. The doctor will do surgery to place a plastic tube called a catheter into your abdomen (belly) to make an access. During the treatment, your abdominal area (called the peritoneal cavity) is slowly filled with dialysate through the catheter. The blood stays in the arteries and veins that line your peritoneal cavity. Extra fluid and waste products are drawn out of your blood and into the dialysate. There are two major kinds of peritoneal dialysis.

What are the different kinds of peritoneal dialysis and how do they work?

There are several kinds of peritoneal dialysis but two major ones are:
Continuous Ambulatory Peritoneal Dialysis (CAPD) and Automated Peritoneal Dialysis (APD).

Continuous Ambulatory Peritoneal Dialysis (CAPD) is the only type of peritoneal dialysis that is done without machines. You do this yourself, usually four or five times a day at home and/or at work. You put a bag of dialysate (about two quarts) into your peritoneal cavity through the catheter. The dialysate stays there for about four or five hours before it is drained back into the bag and thrown away. This is called an exchange. You use a new bag of dialysate each time you do an exchange. While the dialysate is in your peritoneal cavity, you can go about your usual activities at work, at school or at home.

Automated Peritoneal Dialysis (APD) usually is done at home using a special machine called a cycler. This is similar to CAPD except that a number of cycles (exchanges) occur. Each cycle usually lasts 1-1/2 hours and exchanges are done throughout the night while you sleep.

Will dialysis help cure the kidney disease?

No. Dialysis does some of the work of healthy kidneys, but it does not cure your kidney disease. You will need to have dialysis treatments for your whole life unless you are able to get a kidney transplant.

Is dialysis uncomfortable?

You may have some discomfort when the needles are put into your fistula or graft, but most patients have no other problems. The dialysis treatment itself is painless. However, some patients may have a drop in their blood pressure. If this happens, you may feel sick to your stomach, vomit, have a headache or cramps. With frequent treatments, those problems usually go away.

How long has dialysis been available?

Hemodialysis and peritoneal dialysis have been done since the mid 1940's. Dialysis, as a regular treatment, was begun in 1960 and is now a standard treatment all around the world. CAPD began in 1976. Thousands of patients have been helped by these treatments.

How long can you live on dialysis?

We do not yet know how long patients on dialysis will live. We think that some dialysis patients may live as long as people without kidney failure.

Is dialysis expensive?

Yes. Dialysis costs a lot of money. However, the federal government pays 80 percent of all dialysis costs for most patients. Private health insurance or state Medicaid programs also help with the costs.

Do dialysis patients feel normal?

Many patients live normal lives except for the time needed for treatments. Dialysis usually makes you feel better because it helps many of the problems caused by kidney failure. You and your family will need time to get used to dialysis.

Do dialysis patients have to control their diets?

Yes. You may be on a special diet. You may not be able to eat everything you like, and you may need to limit how much you drink. Your diet may vary according to the type of dialysis.

Yes. Dialysis centers are located in every part of the United States and in many foreign countries. The treatment is standardized. You must make an appointment for dialysis treatments at another center before you go. The staff at your center may help you make the appointment.

Can dialysis patients continue to work?

Health: What Is Kidney Dialysis?

Dialysis is the artificial process of eliminating waste (diffusion) and unwanted water (ultrafiltration) from the blood. Our kidneys do this naturally. Some people, however, may have failed or damaged kidneys which cannot carry out the function properly - they may need dialysis.

In other words, dialysis is the artificial replacement for lost kidney function (renal replacement therapy).

Dialysis may be used for patients who have become ill and have acute kidney failure (temporary loss of kidney function), or for fairly stable patients who have permanently lost kidney function (stage 5 chronic kidney disease).

When we are healthy our kidneys regulate our body levels of water and minerals, and remove waste. The kidneys also produce erythropoietin and 1,25-dihydroxycholecalciferol (calcitriol) as part of the endocrine system. Dialysis does not correct the endocrine functions of failed kidneys - it only replaces some kidney functions, such as waste removal and fluid removal.

Dialysis and altitude - A study found that death rates for dialysis patients are 10%-15% lower for those whose homes are higher than 4,000 feet, compared to those who live at sea level.

Some countries, such as the UK, are predicting a doubling of the number of patients on dialysis machine.

Why is dialysis necessary?

Approximately 1,500 liters of blood are filtered by a healthy person's kidneys each day. We could not live if waste products were not removed from our kidneys. People whose kidneys either do not work properly or not at all experience a buildup of waste in their blood. Without dialysis the amount of waste products in the blood would increase and eventually reach levels that would cause coma and death.

Dialysis is also used to rapidly remove toxins or drugs from the blood.

There are two main types of dialysis - hemodialysis and peritoneal dialysis

What type of dialysis a patient should have really does depend on each individual case. Studies have indicated clearly that for kidney disease patients who need to undergo dialysis, one type of treatment is not best for all.

What is hemodialysis?

The blood circulates outside the body of the patient - it goes through a machine that has special filters. The blood comes out of the patient through a catheter (a flexible tube) that is inserted into the vein. The filters do what the kidney's do; they filter out the waste products from the blood. The filtered blood then returns to the patient via another catheter. The patient is, in effect, connected to a kind of artificial kidney.


Patients need to be prepared for hemodialysis. A blood vessel, usually in the arm, needs to be enlarged. Surgery is required for this. The enlarged vein makes the insertion of the catheters possible. US researchers have developed a new way of growing blood vessels using patients' own skin cells to seed the growth of tissue and have tested it in dialysis patients with end stage kidney disease.

Hemodialysis usually lasts about 3 to 4 hours each week. The duration of each session depends on how well the patient's kidneys work, and how much fluid weight the patient has gained between treatments.

In the UK hemodialysis is either done in a special dialysis center in a hospital, or at home. When it is carried out at home it is important that the patient, and/or his/her caregiver knows what to do. Kidney disease patients who are educated about dialysis are more likely to undergo a standard but under-utilized dialysis-related procedure than less knowledgeable patients.

The following people may have hemodialysis done at home:
  • People who can and want to learn how to do it at home.
  • People who are willing to carry on doing it at home.
  • People whose condition has been stable while on dialysis.
  • People who do not suffer from other diseases which would make home hemodialysis unsafe.
  • People who have suitable blood vessels for the insertion of the catheters.
  • People who have a caregiver, and that caregiver is willing to help with hemodialysis.
  • People whose homes can be adapted for hemodialysis equipment.
In the UK, the National Institutes of Health and Clinical Excellence (NICE) recommends that every patient deemed suitable for home dialysis should have both home dialysis and hospital offered.

What is peritoneal dialysis?

A sterile (dialysate) solution rich in minerals and glucose is run through a tube into the peritoneal cavity, the abdominal body cavity around the intestine, where the peritoneal membrane acts as a semi-permeable membrane.

The abdomen is the area between the chest and hips - it contains the stomach, small intestine, large intestine, liver, gallbladder, pancreas and spleen. Peritoneal dialysis uses the natural filtering ability of the peritoneum - the internal lining of the abdomen. In other words, peritoneal dialysis uses the lining of the abdomen as a filter of waste products from the blood.

The dialysate is left there for some time so that it can absorb waste products. Then it is drained out through a tube and discarded. This exchange, or cycle, is generally repeated several times during the day - with an automated system it is often done overnight. The elimination of unwanted water (ultrafiltration) occurs through osmosis - as the dialysis solution has a high concentration of glucose, it results in osmotic pressure which causes the fluid to move from the blood into the dialysate. Consequently, a larger quantity of fluid is drained than introduced.

Although peritoneal dialysis is not as efficient as hemodialysis, it is carried out for longer periods. The net effect in terms of total waste product and salt and water removal is about the same as hemodialysis.

Peritoneal dialysis is done at home by the patient; by a willing and motivated patient. It gives the patient a greater amount of freedom and independence because he/she does not have to come in to the clinic at multiple times each week. It can also be done while traveling with a minimum of specialized equipment. Peritoneal dialysis is said to 'save lives and save money'.

Before having peritoneal dialysis, the patient needs to have a small surgical procedure to insert a catheter into the abdomen. This is kept closed off, except when fluid is being introduced or taken out of the abdomen.

There are two principal types of peritoneal dialysis:
  • Continuous ambulatory peritoneal dialysis (CAPD) - this requires no machinery and can be done by the patient or a caregiver. The dialysate is left in the abdomen for up to eight hours. It is then replaced with a fresh solution straight away. This happens every day, about four to five times per day.

  • Continuous cyclic peritoneal dialysis (CCPD) - a machine does the dialysis fluid exchanges. It is generally done during the night while the patient sleeps. This needs to be done every night. Each session lasts from ten to twelve hours. After spending the night attached to the machine, the majority of people keep fluid inside their abdomen during the day. Some patients may require another exchange during the day. A study found that a significant number of patients prefer "dialysis while you sleep" treatment. Nocturnal dialysis improves heart disease in patients with end-stage kidney failure.
Peritoneal is ideal for patients who may find hemodialysis too exhausting, such as elderly people, babies and children. As it can be done while the patient is traveling it is more convenient for those who have to go to school or to work.

A combination of aspirin and the anti-platelet drug dipyridamole significantly reduce blockages and extend the useful life of new artery-vein access grafts used for hemodialysis.

Dialysis helps, but is not as efficient as the kidneys

Although dialysis helps patients whose kidneys have failed, it is not as efficient as a normal kidney. Consequently, patients on dialysis need to be careful about what and how much they drink and eat. They will also need medications.

A significant number of patients on dialysis can work and lead normal lives. It is possible to go away on vacation as long as dialysis treatment is possible at their destination.

Women on dialysis will probably not be able to get pregnant. There will be a higher level of waste products in the body compared to a woman with normal kidneys - this interferes with fertility. Women who do become pregnant while on dialysis will probably need increased dialysis during the pregnancy. If a woman has a successful kidney transplant her fertility should return to normal. Dialysis has some effect on male fertility, but much less than on female fertility.

What are the symptoms of kidney failure?

Kidney failure tends to happen gradually. Even if just one kidney works, or both work partially, normal kidney function is still possible. So, it can be a very long time before any symptoms are noticed by the patient. When symptoms do occur they tend to be different from person-to-person, making it harder for doctors to diagnose kidney failure quickly. The following symptoms may be present:
  • Fatigue (tiredness)
  • Frequent need to urinate, especially at night. Frequency grows with time
  • Itchy skin
  • Erectile dysfunction (men have difficulty getting and/or sustaining an erection)
  • Nausea
  • Shortness of breath
  • Water retention (swollen feet, hands, ankles)
  • Blood in urine
  • Protein in urine
A sudden injury can cause kidney failure. When it does, symptoms tend to appear faster, and progress more rapidly as well.

Anemia - People with chronic kidney disease are usually affected by anemia (90% of them). When levels of EPO (erythropoietin), which is produced by the kidneys, are low, anemia can develop. EPO makes the body produce red blood cells. When your red blood cell count is low you have anemia. Chronic kidney failure patients who have anemia are usually given an ESA (erythropoiesis-stimulating agent) injection. Ferumoxytol, a novel intravenous form of iron that permits rapid administration of large doses, is effective for treating iron deficiency in chronic kidney disease (CKD) patients on dialysis.

What are the causes of kidney disease?

Dialysis patients' health and lifespans linked to their mental health

Scientists from the Ilsan Hospital in Gyeonggi-do, Korea, and the University of Pittsburgh Medical Center, USA, found that dialysis patients with gradually worsening mental health have a greater risk of having heart problems and dying early, compared to their counterparts with no mental health problems.

The researchers explained that heart disease is the leading cause of early death among patients with kidney disease.

They revealed, in an article published in Clinical Journal of the American Society Nephrology, that dialysis patients with progressively lower mental health scores had a 7.6% higher risk of being admitted to hospital for heart problems, and were 5.8% more likely to suffer a premature death.

Pomegranate Juice May Help Dialysis Patients

Researchers from the Israel Institute of Technology, Haifa, Israel, found that regular pomegranate juice consumption may help dialysis patients ward off a number of complications.

They reported that in a study involving 101 dialysis patients, pomegranate juice reduced the risk of cardiovascular events and infections.

Dialysis patients on pomegranate juice experienced significantly less inflammation and oxidative stress damage caused by free radicals, compared to those on placebo.

If patients drank a "controlled amount of pomegranate juice with a safe and monitored content of potassium", doctors may find that they would have fewer complications. The scientists added that "It is important to consider the risk involved in potassium overload, especially in chronic kidney disease (CKD) patients with dietary potassium restriction."

Story - My Limited Thoughts

I have recently moved from Dubai to Abu Dhabi  due to change in job. My apartment is near to the beach which is great for summer season but worst now a days because extremely cold wind flows in winters specially in the evening and night time. My car parking is around 200 meters from the beach and the apartment is around 300 meters further away from the parking.

Yesterday, I left office after 12 hours of tiring work; parked my car and walked briskly towards my apartment building in cold breeze so as to sleep as soon as possible. After quick dinner, I jumped into the bed and cover myself with blanket. I took my mobile to set the alarm for the morning. I noticed that the mobile battery is running out of charge whereas I left my laptop bag and charger inside the car. Being tired, I started cursing myself on how could I forget to keep the charger with me. I whispered to God in a complaining way on why He wants me to go out to the beach with my tiring body and mind in this much cold time of night. Anyways, I reached my car with half-slept-brain with continuous complaining thoughts. While I was taking the mobile charger,  I noticed that I forgot to place the car parking ticket on the dashboard. The ticket violation is 200AED to 400 AED (up to 11000 PKR or 6800  INR). So I went to parking machine immediately, bought the ticket and placed on the dashboard and felt good that I saved myself from being charged by Police.


Now I realized that why God wanted me to come back to the car. He exchanged my little discomfort with assurance of complying with the Traffic and Parking laws and saving my money. In this ten minutes situation I refreshed my knowledge that God plans everything with His great wisdom which sometimes is beyond our comprehension level. He is the Creator and we are His creation so there is no match at all between His master plans and our plans which we make with our little brains. While we may not fathom the wisdom behind His moves, we need to develop and strengthen the spiritual belief that whatever happens in our lives certainly has a reason behind it. We need understand the fact that God takes us in deep waters not to drown us but to cleanse us. The more stronger the belief we possess the higher the level of peace and contentment we attain in our lives

Instead of feeling stuck, feel love. Give life to that love, and you won't be stuck any longer.

Story: The Fruits of Honesty

An emperor in the Far East was growing old and knew it was time to choose his successor. Instead of choosing one of his assistants or his children, he decided something different. He called young people in the kingdom together one day. He said, “It is time for me to step down and choose the next emperor. I have decided to choose one of you.”
The kids were shocked! But the emperor continued. “I am going to give each one of you a seed today. One very special seed. I want you to plant the seed, water it and come back here one year from today with what you have grown from this one seed. I will then judge the plants that you bring, and the one I choose will be the next emperor!”
One boy named Ling was there that day and he, like the others, received a seed. He went home and excitedly told his mother the story. She helped him get a pot and planting soil, and he planted the seed and watered it carefully. Every day he would water it and watch to see if it had grown. After about three weeks, some of the other youths began to talk about their seeds and the plants that were beginning to grow.
Ling kept checking his seed, but nothing ever grew. Three weeks, 4 weeks, 5 weeks went by. Still nothing. By now, others were talking about their plants but Ling didn’t have a plant, and he felt like a failure. Six months went by–still nothing in Ling’s pot. He just knew he had killed his seed.
Everyone else had trees and tall plants, but he had nothing. Ling didn’t say anything to his friends, however. He just kept waiting for his seed to grow.
A year finally went by and all the youths of the kingdom brought their plants to the emperor for inspection. Ling told his mother that he wasn’t going to take an empty pot. But honest about what happened, Ling felt sick to his stomach, but he knew his mother was right. He took his empty pot to the palace. When Ling arrived, he was amazed at the variety of plants grown by the other youths. They were beautiful–in all shapes and sizes. Ling put his empty pot on the floor and many of the other kinds laughed at him. A few felt sorry for him and just said, “Hey nice try.”
When the emperor arrived, he surveyed the room and greeted the young people. Ling just tried to hide in the back. “My, what great plants, trees and flowers you have grown,” said the emperor. “Today, one of you will be appointed the next emperor!” All of a sudden, the emperor spotted Ling at the back of the room with his empty pot. He ordered his guards to bring him to the front. Ling was terrified. “The emperor knows I’m a failure! Maybe he will have me killed!”
When Ling got to the front, the Emperor asked his name. “My name is Ling,” he replied. All the kids were laughing and making fun of him. The emperor asked everyone to quiet down. He looked at Ling, and then announced to the crowd, “Behold your new emperor! His name is Ling!” Ling couldn’t believe it. Ling couldn’t even grow his seed. How could he be the new emperor? Then the emperor said, “One year ago today, I gave everyone here a seed. I told you to take the seed, plant it, water it, and bring it back to me today. But I gave you all boiled seeds which would not grow. All of you, except Ling, have brought me trees and plants and flowers. When you found that the seed would not grow, you substituted another seed for the one I gave you. Ling was the only one with the courage and honesty to bring me a pot with my seed in it. Therefore, he is the one who will be the new emperor!”

Why Personal branding is important?
In your routine official life, you are being observed by your team lead, managers and/or senior managers on how you work, behave, speak, act, react, meet, greet, resolve and so on. All these observations are contributing towards your overall image. This image analysis is being done by the management for everyone. So when it comes to new opportunity or project in the department, it will be awarded to the one having good professional or corporate image.

Here are the things which you need to work on to promote your corporate image:

1-THINK BIG:
It all starts from your brain. Good Mind Good Find. It is as simple as that. Focusing on your thoughts will give maturity to your speech and actions which consequently would result in great personality. If you think you should only work on the physical appearance factor of your personality by looking nice through branded coats, pants and other accessories; it is going to result in a disaster since it will be equivalent to a bitter toffee in a nice wrap. You can never know for sure what motivates other people. But you can learn what you are accepting or judging in yourself. For instance, if someone makes a remark about you and it's something you also judge in yourself, it will most likely hurt. However if they make the same remark and you don't have that judgment about yourself, it probably won't bother you at all.

I once visited a new friend's house and everyone in the family was shorter than me. Since I'm the shortest person in my family, I never felt too tall.
When my friend's mother met me at the door and said with a slightly disappointed tone, "Oh, you are so tall," it didn't affect me. I was aware that she had some discomfort with my height, but I didn't take it personally.
However, had she been tall and said, "Oh, you are so short," it probably would have pushed my buttons, since I do feel somewhat short.

This point is valid for almost any interaction imaginable: Reactions always have to do with our own self-judgments and feelings of inadequacy or strength, not the other person.

Most judgments of others stem from one of three basic causes:

1- You wouldn't tolerate the same behavior or characteristic in yourself. For instance, you might be shy and encounter a very gregarious person.

2- Your judgment might go something like this: What a show-off. They are so loud and obnoxious. Because you would be embarrassed to act this way, you resent somebody else doing it. This type of judgment might reveal that you are not fully expressing yourself, hence you feel resentful or put off by others doing so, even if they do it clumsily. Becoming aware of the truth of this reaction and working on expressing yourself more fully and authentically would result in a valuable gift of freer self-expression.

3- You display the same behavior and aren't aware of it so you project your disowned behavior onto others and dislike it "out there."
Everyone has encountered the second cause at some point. Someone is complaining about a friend or acquaintance and you think to yourself,  "That's funny, they do the same thing they are finding wrong!"

Taking an honest look within to see if you share some of the characteristics you dislike in others. You may be surprise to learn that you do, and it is likely to offer insight into gaining greater self-acceptance and compassion for others.

You are envious and resent the feelings that come up so you find something wrong with those who have what you want and end up judging them.
Someone who has attained recognition may remind you of your own lack of success in this area. You may resent their accomplishment and then find something wrong with them in order to avoid your own feelings of inadequacy.
Since inspiration is a much more effective motivator than competition, you'd be more likely to experience success if you got inspired by other people's victories instead of wasting time finding fault with them.Most judgments of others are ego strategies to avoid uncomfortable feelings. However, if you lack the awareness of where they come from, they can lead to even more discomfort down the line.


Becoming aware of the nature of your judgments doesn't mean that you no longer have preferences. You may still notice that certain types of behavior seem unappealing. But with right understanding and a little work, discernment rather than judgment kicks in and causes you to feel compassion for others, even if you're not enthusiastic about their behavior. At the very least, you'll feel neutral.

Discernment is awareness and understanding without an emotional response. Exercising discernment feels very different from getting your buttons pushed. Judgments that cause emotional reactions are clues to help you find personal insight.

When you explore beliefs and assumptions instead of judging people, you open a door to expanded self-awareness and self-acceptance.
Rather than unconsciously delighting in the ego gratification of judging others, let your reactions and judgments help you achieve greater self-understanding—and accordingly, greater happiness and success.

When you use your judgment of others as a mirror to show you the workings of your own mind, every person's reflection can become a valuable gift, making each person you encounter a teacher and a blessing.

When couples first get together, everything is new and exciting. They overlook the little annoying things the other person does. However, after time, the nagging starts, instead of hearing, You look beautiful, they might hear Why are you wearing that shirt? If this sounds like your relationship, first, the two of you need to sit down and be honest that things have changed. Identify the things each other did in the beginning of the relationship that created the attraction in the first place. Then together, make a commitment to start over. The truth is, both of you will have to work on this. It will not automatically be easy but it is possible. Start by forgiving each other, forgetting the past, and then start over with the flirtation. Focus only on the special things your mate does and relearn to put the unimportant things aside. It will take some time so be patient.

When I was holding the Wimbledon Championship Trophy I never asked God "Why Me?" So I have no right to ask God "why me?" during my times of hardship."
Isn't it true that a human being often forgets God during times of happiness? Whenever he achieves something in his life he attributes it to himself, his hard work and support from his family and friends and forgets to thank God. But what does the same person do when he is afflicted by a misfortune? The same person who takes credit for every achievement of his now starts blaming God for his
misfortune.
Moral: God keeps reminding people of His existence by giving them trials and afflictions, because people tend to forget God during their good times and such people only become aware of God if harm touches them. Never forget God in your good times. And if you are going through bad times, thank God even more for the times could have been worse.
